Diagnosis
of IBS.
Irritable
Bowel Syndrome belongs to long list of unexplained
conditions of the gut. Frustrated experts grouped them together and
called them functional.
If
you have IBS, then your pain, diarrhoea and/or constipation, and
bloating are functional. In other words, your symptoms cannot be
explained by current medical tests.
Irritable
Bowel Syndrome
is included in the category MUD (no kidding) Medically Unexplained
Disease or Disorder. Included in this category is fibromyalgia.
The
Manning Criteria was established in 1978 following a study of Irritable
Bowel Syndrome symptoms.
A patient has a diagnosis of
Irritable Bowel Syndrome if there is
abdominal pain and at least two of the six listed below in the absence
of any structural disease.
- Abdominal pain eased
after defecation.
- More frequent bowel
movement at onset of pain.
- Loose stools at onset
of
pain.
- Abdominal distension.
- Mucus with stools.
- Feeling of incomplete
emptying.
Manning et al 1978.
A team of Gastro-enterologist met in Rome and produced the
definition and symptom criteria for Irritable Bowel Syndrome.
It is known today as the Rome II Criteria.
From these Criteria Irritable Bowel Syndrome is the diagnosis when
there is duration of 12 weeks or more of abdominal pain or discomfort
with two of the listed three features in the absence of structural or
other explanation.
- Pain relief
with defecation.
- Change in
frequency of stools.
- Change in the
form of stools.
The
more symptoms, the easier the Diagnosis of Irritable Bowel Syndrome.
Abnormal
stool frequency is more than 3 times a day (diarrhoea) and less than 3
a week (constipation)
Stool
form is abnormal if it is lumpy or hard, or loose and watery.
Stool
passage is abnormal if there is straining, urgency, or feeling of
incomplete evacuation.
Passage
of mucus as a rule is not common.
Bloating
or feeling of distension of abdomen is a distressing symptom.
The
Rome II Criteria was designed to help family doctors to reach a
diagnossis of
Irritable Bowel Syndrome confidently with few tests. This is
particularly helpful in children with Irritable Bowel Syndrome and
avoids them
having embarrassing and unpleasant invasive investigations such as
Sigmoidoscopy and Colonoscopy.
